Webinar: Predicting the Next COVID-X: Preparedness for Future Infectious Threats

1. The trainee and/or the sponsoring company (“the Company”) hereby acknowledges and agrees that Parkway College of Nursing & Allied Health Pte. Ltd. (“Parkway College”) is authorised to collect, use, and disclose the personal data of the trainee for the purposes set out below and other reasonably related purposes, in compliance with the Personal Data Protection Act 2012 (“PDPA”) and the IHH Singapore Personal Data Protection Notice:
- Registration for courses;
- Administration and servicing of course-related activities;
- Certification and accreditation processes;
- Administration of course surveys;
- Application for course-related funding (where applicable), including disclosure to relevant funding agencies, bodies, or organisations (such as SSG, WSG, AIC, etc.);
- Marketing and promotion of Parkway College’s products and services (where the trainee has expressly opted in).
The Company further represents and warrants that each trainee has provided valid and informed consent to the collection, use, and disclosure of their personal data by Parkway College, in accordance with the IHH Singapore Personal Data Protection Notice.

2. Parkway College is committed to maintain the confidentiality of the personal information and undertakes not to reveal applicant's information to any third party (other than the funding-related agency) without prior consent of the trainee. 3. The application form must be submitted to Parkway College at least 14 days before course commencement.

4. All prices for the courses are subjected to GST (Goods and Services Tax).

5. The course schedule selected is subject to confirmation. Parkway College reserves the right to cancel or re-schedule the class due to any unforeseen circumstances or low enrolment.

6. All courses will be conducted in English. Trainee must be able to understand, converse and write in English.

7. Request of Replacement/Withdrawal of Courses
- For withdrawal of course, a written notice must be submitted and received by Parkway College 14 days before course commencement.
- For replacement of course, a written notice, together with the full particulars of the new trainee, must be submitted and received by Parkway College 7 days before course commencement.
- There is a penalty fee for withdrawal:
o 50% of the course fee if a withdrawal letter is received less than 14 days before the course commences.
o 100% of the course fee if a withdrawal letter is received on or after the course commences or no show.

8. Mode of Payment
- PayNow, telegraphic transfer or cheque made payable to “PARKWAY COL OF N&A".

9. Parkway College reserves the right to claw back the funded amount from trainee if the trainee did not meet the following eligibility criteria:
- Did not meet the attendance requirement (>75%); and/or
- Did not submit *correct UEN during the enrolment for company sponsored trainee; and/or 
- Did not attain “Competent” at the end of the course.
Note: The UEN provided must be the sponsoring employer who is (i) the direct employer of the trainee, and (ii) the party providing salary and other statutory benefits (such as CPF contribution, leave entitlement, etc). The UEN will be used to verify the employment status of the trainee as part of the grant disbursement process.
